New Client Intake

Welcome to Adapt Transition Services! We serve clients with autism, ages 14+, via home, in center treatment plans, or recreational programs.  

Translation services will be provided to any recipient, when necessary and requested. The interpreter will assist with translating any intake, treatment plans, evaluation, or other documents shared with the family. Adapt Transition Services also utilizes the MA Association of the Deaf if required by the client.

To get started, fill out an interest form by clicking one of the buttons below. Once received, a team member will contact you about next steps.

We accept the following funding sources: Self-pay, Blue Cross Blue Shield, Tufts Health Plan, MassHealth, Beacon, and additional insurances as applicable, however our preferred methods are self-pay or Blue Cross Blue Shield. If we are unable to service you at this time, we will include you on a waitlist. We welcome you to complete the new client intake form for home and center treatment plans. 

Additionally, please get in touch with your insurance plan for information regarding assessment tool requirements and when those assessments were performed, e.g., Autism Diagnostic Observation Schedule (ADOS), Autism Diagnostic Interview (ADI-R), Parent Evaluation Developmental Stages (PEDS), or Brigance Diagnostic Inventory of Early Development II, CARS, MCHAT. This is key data we need as part of intake. 

For those interested in participating in recreational programs (open to those not currently receiving insurance-based services at Adapt), please complete the following separate form.
