Reason

When an individual with a disability turns twenty-two, they age out of school and enter adulthood. At this time, they lose services that were mandated to them, and enter adult services. Because of this loss, it is critical that skills are developed during the transition period to promote successful adulthood outcomes.

 We have identified crucial aspects needed for successful transition. During the transitional period, individuals need to be provided with inclusion opportunities to foster growth, employment opportunities with on the job training, daily living and social skills training to promote independence and success in adult life.

Mission

We will identify skill deficits, and develop individualized, evidence-based programming to improve adaptive functioning. We will create behavior support plans to increase desired and socially significant behavior. With this, we will be able to maximize community integration and focus on skills needed for adulthood.

Adapt Transition Services practices the "Happy, Relaxed, Engaged" (HRE) philosophy. With this approach, we strive to continually learn about the individual being served, so that we are able to create an environment which supports the development of socially significant skills. We learn by listening to what interfering behaviors are telling us, creating an environment where an individual feels safe and in control, letting the individual take the lead in the process, and empowering them to engage in behaviors which maximize their contact with reinforcement.


At Adapt TS, we listen, learn, guide, and coach. We prioritize our clients well being. and believe that when we do this, we can best promote independence and support them on their path to adulthood.
